Guidelines to Enter Thailand / Visa Application Procedures

  • Please make sure that you are in possession of a passport valid for at least 6 months.
  • a round-trip air ticket
  • Vaccination Certificate or RT-PCR / Pro-ATK test result.
  • Printed Travel Insurance certificate
  • Copy of confirmed hotel booking
  • Copy of confirmed flight reservation

visa age

  • 3 months (Single Entries)
  • 12 months (Multiple Entries)

Period of stay in the Kingdom

  • 60 days per time

supporting documents

  • Passport or document used in lieu of a passport Service life not less than 6 months passport valid for at least 6 months.
  • Completed visa form
  • Two 2 ½-inch photographs (taken within the last 6 months / without hats and sunglasses)

Visa Extension – In the case of receiving medical treatment

 

In the case of receiving medical treatment, attending rehabilitation, or taking care of a patient:

  1. Must have been confirmed and requested by a physician of the hospital providing said treatment with full explanation about the illness, period of treatment, and the physician’s opinion that the illness is an impediment to travel.
  2.  In the case of taking care of a patient, the applicant must have been confirmed and requested by a physician of the hospital providing the examination and treatment or by an embassy or consulate.
  3. In the case of caretakers who are not the parents, spouse, children, adopted children, or spouse’s children, permission shall be granted for no more than one person.Documents to be submitted

Document Checklist in Case of Receiving Medical Treatment

  1. Application Form (TM.7) with 1 photograph by 4X6 cm. (Link)
  2. Copy of applicant’s passport

– Front Page  of passport  showing name / surname / Passport No., ect.

– current visa with latest arrival stamp from immigration checkpoint 

– Departure Card (TM.6)

  1. Certified Documents from the Hospital

– Letter of confirmation and request for a temporary stay issued by the physician of the hospital providing treatment.

– Photo of patient while receiving a treatment in the hospital

Receipt of Residence (TM.30)

  1. POWER OF ATTORNEY ( in case Have relatives do it instead )

– A copy of grantee’s passport (Front Page)

  1. In the case of taking care of a patient, the applicant must attach a letter of confirmation and request for a temporary stay issued by        a physician of the hospital providing said treatment and documents stating relationship (in the case that the caretaker is a member of the family), such as a marriage certificate, a birth certificate, registration of child legitimization, child adoption registration certificate, or confirmation letter from the embassy or consulate.
  2. Visa Extension Fee 1,900 THB
